Formfilling service from Twyford Surgery
Twyford Surgery Patient Participation Group
The PPG have set up a formfilling service for the healthcare professionals at the Surgery to offer to patients. A Guide to Eligibility has been created in the case of Attendance Allowance applications for those over the age of 65 with medical conditions. The guide also offers some information on Universal Credit for those between the ages of 16 and 65 (or relevant State Retirement age)
A member of the PPG committee has been volunteering for Age Concern Hampshire for 4 years and has completed over 50 forms with only 2 failures. It is hoped that this service will help those patients who need assistance to remain independent in their own homes.
