Neil Johnson is collating feedback on issues with accessing GP services in Farnsfield  for the Health Scrutiny Committee of Notts County Council. 

If you would like to contribute please email neilppg3@gmail.com giving a brief description of the complaint or compliment, the date of the incident and if the concern has been previously raised with the practice.  Feedback can be anonymous. Try to use one of the following headings

  • Reception and Admin. Staff
  • Availability of appointments
  • Prescriptions
  • Home visits
  • Minor injuries
  • Health care professionals (doctors and nurses)
  • Referrals including patient notes and results
  • Responses to complaints
  • Blood tests and health checks
  • Other.

 

In addition to feedback provided to Neil , the NHS use a feedback form to evaluate patient experience called the “Friends and Family Test”. This is available in the Surgery waiting room and also from Neil Johnson.

Feedback can also be posted to the Village Centre mail box, or returned to Neil in writing.
