Availability Form

Please use this form to provide details of your availability for friendly matches. This information will be automatically forwarded to the Men's Captain.

  • For those matches that you know you will be available for, select Available.
  • For those matches that you know you will not be available for, select Unavailable.
  • If you are not sure yet, just leave the match as Select... 

Once the team has been selected for a match, please use this form to show that you are still able to play by choosing Confirmed or Confirmed and Lift Needed, as appropriate.

Remember to enter your name at the top of the form, where you can also provide a message for the Captain.

Once you have filled in all the information that you want to send, click on the Submit button at the bottom of the page.

If your availability changes, just return to this page and submit the new information.
